That doesn't look good. Can you see any signs of trauma and is he lame on either leg? Best case is a bit of cellulitis and you can bring it down with a sweat wrap. If he's really lame on that leg or you see a puncture I'd give the vet a call.

ETA- Just saw he's not lame. To get the swelling down cold hosing 20 plus minutes is great then, if it was my horse, I'd slather on a thin layer of furicin all over the swollen areas, cover with seran wrap then a quilt and stable bandage pretty snug or it will slip as the swelling goes down. Leave it on several hours of overnight then cold hose the goo off and see how much the swelling went down. If the swelling is gone I may do the quilt wrap one more night for support and cold hose again.
