Although I never drove after drinking, the "reality"of it never really crossed my mind until for a school fieldtrip we went to a hospital. There, we talked to people who were my age, and a bit older, but who had driven drunk or got in a vehicle with someone who had and as a result they were now brain damaged, horribly scarred, paralyzed, etc...
Just listening to their stories had such a profound effect on me, that one day they were out drinking and having a good time, and the next they were bound to a wheel chair for life mourning the death of their friend, over something so stupid.
