I've noticed that if an owner is completely, 100% clueless and has trash/debris/wire/cars/tractor parts/unsafe fences in the pasture, their horses seem to rarely get hurt - but a overly careful owner will often have a horse get injuries from nothing at all.

It's very weird.
I've noticed that! I had a horse once that came in with a puncture in his knee. It was no big deal, and healed quickly, but I scoured his clean, clear, field with beautiful 3-board fencing, and couldn't find so much as a stray nail. A few weeks later, another horse came in with a puncture in the same place. Never did find out what they were doing.
