If it goes into the carotid instead of the jugular, yes. Also, some horses (very very rare) can have allergic reactions to some sedatives.

Too right!   What with botched administration, inaccurate doseage and incorrect sedative and anaesthetic and clueless undemanding owners and incompetent vets I'm reading lots of things here that makes me think it's no wonder that the horse gets to the point where it requires euthanasia!
