I don't know why everyone keeps insisting that spay incontinence isn't that common. It's extremely common. Maybe just in my breed but I know more spayed bitches with incontinence (including Amara) than without. It is a PITA, smelly, dirty and the pills can make them loopy and all fucked up.

I'm kinda with TRD. KOTJ has done a lot right by this dog and has taken lots of advice given to her. It's her choice and I definitely see the reasoning behind her decision. If Maddie ends up pregnant, which I'm willing to bet she wouldn't be taking her out to the barn in heat because that would just be obnoxious and carrying around a bleeding dog in your car doesn't sound like much fun, then she can go ahead and have her spayed because it will prove she couldn't keep her from being pregnant and eliminate the problem at the same time.

At the same time, having an intact male is a lot different from an intact female. I got Amara spayed before Diesel got neutered because I didn't want to deal with the issues. I also don't think that KOTJ is as careless about Maddie as some think she is and believe she should have benefit of the doubt while taking care of her.

Spaying is ROUTINE surgery. Doesn't necessarily make it a "safe" surgery. All surgery is risky and if she doesn't want to risk her dog, that's her decision.
