He seems like a prime candidate for impaction colic,If you can't get to a vet or have one come out,try the enema


here is a small snip from a medical book-
Usually,the impaction builds up gradually,producing subacute adominal pain for 8-12 hours.
There will be mild signs of pain and only a slight increase in pulse and respiration.
This is followed by an acute phase,if the impaction is in the ileocaecal area.
The acute phase is characterized by an increase in the severity of the pain.
Severe depression,patchy itchy sweating,coldness of the extremities and violent rolling and struggling accompany the acute pain.
At this point, the abdominal pain becomes severe and continuous, raising the pulse rate to between 80 and 120 beats per minute.
There is also an increased respiratory rate of 30-40 bpm and a temp of 103,death usually occurs 36-48 hours after the onset of the illness when the ileocaecal is impacted.

When only the colon is impactedthe disorder is prolonged and less acute,bouts of pain occur at 1/2 hour intervals and are of moderate severity,the horse does not eat but may drink or lap water almost constantly.
Pulse rate does not raise above 50 bpm and temp and respiration may not be affected, Feces are passed in small amounts
that appear hard and covered in a thick sticky mucus.
