A horse will not lay down & get up repeatedly if they're not colicing. That's the big difference.

Too many people wait till it's too late to do anything & save the horse. As soon as I seen one of mine showing any signs of mild colic I give them an enema. I don't wait till surgery is needed to try to save them as more times than not they will not make it if it gets to that point.

Giving an warm water enema will not hurt any horse even if it's not from impaction. If enemas were that dangerous they'd be tons of people killing over from taking them. You just do not want to ever use a garden hose or any tube end that has a ruff non-smooth end.
