Jordie - my mare had a similar looking injury, but on her neck. It didn't start off the same, because she just ripped off the skin, leaving exposed muscle, but your pictures reminded me of it. She was off at a breeders and did this somehow.

I scrubbed it twice a day with a sponge & ivory bar soap, and covered it in AluShield.

Wound progression:

After vet cleaned & clipped the hair off the edges. The wound edges weren't touched, we have NO idea what she did. The hole in the bottom right corner was about 1/4 inch from her jugular, you could see it pulsing. If she'd ripped it a little bit more, she'd have been dead before the breeder even saw her.

There was only a little bit of visible scarring, mostly seen because of the hair growing in a bit different.
